EMERGENCY MEDICAL AUTHORIZATION
To maintain current information on contacting parents in case of student medical emergencies, we will be asking parents to complete an online emergency medical authorization form. This will replace the paper Emergency Medical Authorization form, a blue card and a white card, but instead you will go to the link below and fill out one form on-line which will replace the White card, Blue card and EMA.
If you have more than one child, you can copy the forms on-line and edit the places that are different for each child.

Once you are on the page, follow these steps:
- Enter your Progress Book username and password and click login.
If your student is new to the district, parents will receive this login information in the mail by September 1, 2009. - Click on the “Student Emergency Medical Information Form” link for your child.
- Some areas are already filled in for you; any area that has a red dot must be filled in.
- The person/people we have listed as legal guardians will already be entered, please click the “Add Contact” link to add any additional contacts for us and click save.
- If you give consent for Emergency Medical Treatment, you will need to select that in the consent section. Once you select “I give consent,” you will see a drop down screen to add medical information. Please add the requested information.
- If your child has allergies, or a medical history that we need to be aware of, please click the “I want to add medical history/allergies” checkbox and add the pertinent information in that area.
- When you get to the end, click “submit form.” You will notice that your form has “complete” listed next to it when you successfully complete it.
- If you have more than one child, and their progress book accounts are tied together, repeat the process, but you can select the “copy form” icon under the action column and then add any pertinent medical information in the appropriate section.
- If their accounts are not tied together, you will need to log in to the formshare site again with your next child’s username and password and complete the process again.
